Source & methodology
Data source. Results come from the official CMS NPPES Read API, version 2.1. NumPad sends the lookup through its HTTPS proxy and formats the API response; it does not create or independently verify the provider data.
Displayed fields. Depending on what NPPES returns, NumPad shows the NPI, enumeration type, NPI record status, enumeration/update/certification and deactivation/reactivation dates, public names, taxonomy records, reported license fields, mailing and practice locations, other identifiers, organization other names, and digital endpoints.
Important limits. CMS explains that NPI issuance does not ensure licensure or credentialing, guarantee payment, or enroll a provider in a health plan. NPPES data are reported by providers or authorized representatives. Verify high-stakes decisions with the appropriate licensing board, payer, credentialing source, or program.
